A dying Thakur Surjeet Singh bequeaths the entire estate to his son, Raghuvir, disowning the other, Dhurjan, the family's black sheep, who also indulges in black magic. The latter swears to use magical powers to usurp the estate, and even arranges the abduction of Raghuvir's daughters, Sapna and Aarti. Mangal and his men manage to apprehend Dhurjan, imprison him in a dungeon, and rescue Aarti. However, they are unable to locate Sapna, and Raghuvir is killed. Before dying, he informs Mangal that Sapna has one of two pieces of a locket around her neck, while the other is on Aarti's, and when joined together will reveal the location of a treasure buried in a dungeon. 20 years later, Aarti and her boyfriend, Vijay, along with several others, will attempt to unearth this treasure - not realizing that they not only face betrayal from one of their own, but will also release and fall prey to an ageless and indestructible entity.

The film opens with a scene taking place some 200 years ago, with the royal procession of Raja Harimansingh of the sultanate of Bijapur, stranded near the Kali Pahari (literally, black mountain). The Raja is concerned because his daughter Princess Rupali has disappeared near the lair of the devil-worshipper Samri (Anirudh Agarwal). The princess wanders into the ruins of an old fortress and is promptly captured and tortured by the villainous Samri. His trademark attack is mesmerizing the hapless victim apparently sucking out their life force through the eyes, causing their natural eyes to be replaced with demonic white shades. During this process, Samri's eyes gleam blood red. Raja Harimansingh catches Samri in this terrible act and orders the soldiers to capture him.

A grotesque brougham stops in a dark forest and its cowled driver, Lalbai the sorceress, beckons to the bewitched passenger, a newly wed bride. The film opens with a young couple Raja (Anil Dhawan) and Rita (Priti Sapru) are driving through a dense forest at night. Tired and sleepy, they decide to stop for the night, and find a haveli (mansion) nearby. They fall asleep in the outskirts of the mansion. Around midnight, the husband awakes all of a sudden and gets up to investigate. Then a horrible monster (Manik Irani) attacks him. His wife wakes up hearing him scream and they are both killed by the monster. An old man Narendra (Narendra Nath) suddenly appears, a bit too late but eventually traps the monster in an underground cage and attaches a cross to the latch to keep him from escaping.

Kouga Saezima travels to a city from his childhood after receiving orders to hunt down the Horror Karma, an ancient female Horror who is one of the infamous Apostle Horrors. By that time, Karma's followers, Kurusu and Shion set up base in a gothic club called Crime to satisfy their mistress's appetite by bringing her various girls and lure them into her realm with false promises of making their dreams a reality. As Kouga arrives, ignoring the memories he has of it, a Makai Priestess named Rekka also arrives in the city and aids a pair of Makai Priests named Akaza and Shiguto in saving a baby from a Horror posing as a mother. However, Kouga arrives and reveals the "baby" to be a Horror that Zaruba identifies as the cannibalistic Apostle Horror Babel. Though Rekka meddles in the fight, Kouga managed to kept her aside before donning his armor and slaying Babel. As he leaves, Kouga is approached by Akaza as he introduces him and his associates to him and Zaruba. When Kouga reveals his mission to find and destroy Karma, Rekka reveals having a personal vendetta against Horror and refuses to allow anyone to rob her of her chance to kill the fiend. However, Kouga and Zaruba are unable to find Karma despite the evil presence they are feeling around the city.
