The film revolves around a successful advertising campaign manager named Arjun Dutt (Siddharth). During a routine medical check-up, he discovers that he is HIV positive. He is totally devastated. Instead of facing the truth and telling his pregnant wife and son, he runs away from his home and job.

"To Touch the Soul" follows California State University, Long Beach Professor Carlos Silveira, a Brazilian-born artist educator and social activist who recruits 27 American university students to join him in a pilot program that uses art to help impoverished Cambodian children affected by HIV/AIDS express their wishes and desires for their futures. As Carlos and the students grapple with the realities of a culture much different from their own, a language they don't understand, art projects that don't go as planned and a three-week deadline, they form a bond with the children. Through these young Cambodian mentors—all of them abandoned by society—the Americans empower their own social activism and learn the true meaning of kindness, selflessness, courage and community.

The film opens in medias res, with 22-year-old AIDS activist Pedro Zamora in his New York City hotel room in 1994, getting ready for an appearance on CBS. His contact calls him to ask why he has not arrived at the studio, but Zamora, disoriented, does not know who he is. Hotel security subsequently finds him lying unconscious on the floor.
The film largely proceeds chronologically with Kwan serving both as the person being interviewed and as the narrator. Her colleagues in Hollywood were generally interviewed in Los Angeles. Her family members and companions in her youth were interviewed in Hong Kong.
